WSIB REPORTING
In All Cases
of Injury/Disease
Regardless
of the nature of any injury or disease you may incur on set, it is extremely
important that it be reported immediately to the Workplace Safety &
Insurance Board or Workers Compensation Board
in your Province. Employers (production) are no longer solely responsible
for submitting this claim. It is up to you to be sure it has been sent in.
Delays in reporting may result in a delay or denial of a Workers Claim. The
procedure in which each province deals with Workers Compensation can be different.
You will need to contact the Board in your province to be certain of the procedure.

Reporting
An Injury or Disease in the Workplace In Ontario:
An example of what is expected when reporting an injury/disease in the workplace,
as it pertains to Ontario is as follows:
The Employer
Must:
- Make sure first aid is given immediately.
- Make sure there is a record of the first aid treatment/advice given to
the worker.
- Complete and give to the worker the Treatment Memorandum Form 156 if
the worker needs more than first aid treatment/advice.
- Provide immediate transportation to a hospital, a doctor's office, or
the worker's home, if necessary.
- Complete Form 7 (the Employers Report of Injury Form), if the worker
needs more than first aid treatment/advice. Make sure also to ask the worker
to file a claim and give consent to the release of functional abilities
information on this Form 7.
- Give the worker the pink copy of the Form 7.
- Return the completed Form 7 to the Workplace Safety and Insurance Board
(WSIB) within three days of learning of the work-related injury or occupational
disease. If the worker is unable or unwilling to sign, send the Form on
without the signature.
- Pay full wages and benefits for the day or shift on which the injury
occurred.
- Cooperate in the worker's early and safe return to work.
- Supply a Functional Abilities Form 2647 to the health professional treating
the worker. Ensure that the worker's signed consent to the release of functional
abilities information is attached. This signed consent will either be on
the Form 7 (Employer's Report of Injury) or on the employer's copy of the
Form 6 (Employees Report of Injury).
The Worker
Must:
- Get first aid right away.
- Tell the employer of any injury or the possible onset of a work-related
disease/condition, regardless of the nature or impact of the injury or disease/condition.
- File a claim for workplace insurance benefits promptly, when more than
first aid treatment/advice is needed. At the same time, give written consent
to the release of functional abilities information to the employer. This
can be done by either signing Form 7, if possible, or Form 6 supplied by
the Workplace Safety and Insurance Board (WSIB).
- If signing a Form 6, give a copy of it to the employer.
- Choose a doctor or other qualified health professional. Do not change
health professionals without permission from the WSIB.
- Cooperate in health care treatment.
- Cooperate in safe return to work.
- Complete and return all WSIB form promptly.
- Report to the WSIB any changes in income, return to work status or medical
condition.
Employers'
Report of Injury/Disease Form 7
Form
7 must be completed and sent to the Workplace Safety and Insurance Board (WSIB)
within three days of learning of a work-related injury or occupational disease
that either causes a worker to:
- be absent from regular work;
- require modified duties at regular pay or less than regular pay;
- earn less than regular pay at regular work; and/or
- obtain health care.
Form 7 must also be completed if eye glasses, dentures and/or artificial
appliances were damaged as a result of work-related injuries.
To receive benefits, the worker must file a claim and provide consent to
the release of functional abilities information. This should be done on
the Form 7. If the worker does not sign Form 7, the WSIB will send a Form
6 to the worker to complete.
